Know about Chronic Daily Headache

If a patient suffers from headache for 15 days or more in a month, the term “Chronic Daily Headache” or CDH is used. Diagnosis of Chronic Daily Headache is also done based on this observation i.e. headache for15 days or more in a month. Chronic Daily Headache is a very broad term (not a single entity in itself) and include a number of different headache syndromes, such as chronic TTH (tension-type headache) and headache syndromes that occurs secondary to trauma, infection, medication overuse/overdose, inflammation, and many other causes of headache.

Approximately 4% of adult population suffers from Chronic Daily Headache or headache that is nearer to Chronic Daily Headache, than any other type of headache syndromes. Chronic Daily Headache may be primary or may be secondary to other pathology. The treatment/management of Chronic Daily Headache depends on the primary and secondary nature of the cause. The secondary type of CDH can be treated by treating the underlying pathology which is causing headache.

Chronic Daily Headache, Secondary causes:

The secondary causes of Chronic Daily Headache include posttraumatic, inflammatory, chronic CNS infection and medication-overuse (commonly due to analgesic overuse). Posttraumatic causes of Chronic Daily Headache include head injury, post-infectious, iatrogenic (hospital acquired) etc. The inflammatory causes of Chronic Daily Headache include Giant cell arteritis, Behçet’s syndrome, Sarcoidosis etc.

Chronic Daily Headache, Primary Causes:

Causes of CDH that lasts for less than 4 hours a day:

Chronic cluster headache (a form of CDH), SUNCT/SUNA ( SUNCT–short-lasting unilateral neuralgiform headache attacks with conjunctival injection and tearing and SUNA –short-lasting unilateral neuralgiform headache attacks with cranial autonomic symptoms), chronic paroxysmal hemicrania, hypnic headache etc. are common causes of primary CDH.

Causes of CDH that lasts for more than 4 hours a day:

Chronic migraine, new daily persistent headache, chronic tension-type headache (TTH), hemicrania continua etc. are the common causes of CDH lasting for more than 4 hours a day.
